It is the responsibility of each student to monitor and update all health requirements, CPR certification, & background check information prior to beginning any clinical rotation. Failure to submit and/or provide documented evidence of any out-dated health requirements, CPR certification, and/or criminal background check information SHALL result in the student receiving a 1.0 Critical Incident for each clinical day missed until all documented information is submitted. Please note that more than 2 Critical Incidence points SHALL result in an F in the given clinical rotation which subsequently results in an F in the course.
Students are required to show coverage for the entire semester of enrollment at the beginning of each term for the following: immunization, CPR certification, TB skin test or chest x-ray, liability insurance, major medical insurance, and criminal background check information. Students must submit a photocopy of documentation. Registered Nurses and Licensed Practical Nurses must submit a copy of their current nursing license. Students are encouraged to keep originals or copies of health documentation for their own files. The School of Nursing and Health Sciences must have a signed release from the student to forward documents to an employer or prospective employer.
