Policy Provisions

When a student or faculty member who is being served by the Director of Counseling/Disability Services makes contact with the office in person or by phone, a Documentation Contact Form (Appendix K) should be completed by the office staff member with whom contact is made. This helps to track requests made by students and faculty members and ensures a greater level of accountability of staff members to meet student needs on an ongoing basis.
